Description of the program

The one-year Advanced Clinical Training Fellowship entails comprehensive training in all aspects of minimally invasive urology. This includes techniques for PCNL (Percutaneous Nephrolithotomy), both in supine and prone positions, as well as mini-PCNL and combined approaches. Ureteroscopy and RIRS (Retrograde Intrarenal Surgery) techniques, as well as Shock Wave Lithotripsy, are covered extensively. The program also offers intensive training in HOLEP (Holmium Laser Enucleation of the Prostate) and other transurethral techniques for the treatment of BPH (Benign Prostatic Hyperplasia) and bladder TCC including Laser  En Block Resection. Additionally, the fellow will participate in prostate Fusion Biopsy and focal therapy for prostate cancer. The fellow will be exposed to an integrative approach to stone disease, including detailed metabolic evaluation and preventive measures. Participation in clinical as well as basic research projects is encouraged.

The urology department at Rabin Medical Center is the largest urology department in Israel. The medical center is situated in Petach Tikva, a city approximately 30 minutes' drive from Tel Aviv.

This fellowship program is intended for urologists who have completed their training and desire to specialize in minimally invasive techniques for urinary calculi disease and prostatic surgery. The program's mission is to offer intensive surgical training opportunities to urologists from our region as well as other countries.

Clinical Responsibilities: The fellow will be responsible for conducting daily rounds with the endourology resident. The fellow will see patients in the outpatient clinic, participate in endoscopic surgery, and engage in teaching activities. They will also be responsible for maintaining a prospective database.

Fellowship description Asklepios Klinik Barmbek

We are dealing with the entire range of endourological procedures in our department:

Stone management is fully covered since we have a case load of about 1000 ureteroscopic, percutaneous, and SWL procedures.

We are a reference centre for transurethral thulium laser enucleation of the prostate (worldwide first description of the ThuVEP procedure).

Urothelial tumors of the lower and upper urinary tract are treated by TURB and ureteroscopy.

We perform the following laparoscopic procedures: cyst decortications, nephrectomies, partial nephrectomies, pyeloplasties, ureteral reconstructions, varicocele ligations, and adrenalectomies.

We do not perform robotic surgery.

Our research is mainly based upon in vitro stone research, prospective/retrospective studies on thulium laser surgery and ureteroscopy, respectively.

We have a laboratory with the complete armamentarium for in vitro studies on stone treatment including laparoscopic and endourological training models (HiFi and LowFi simulators)

The fellowship is a 1-year engagement and consists of both research and clinical activities.

Please be aware that our department cannot provide a salary and that you should ensure your own funding.

A certificate of proficiency in the German language has to be done during the time of the programme.

Brown’s Minimally Invasive Urologic Surgery Fellowship has multiple tracks and has evolved over the years and and has adapted to the everchanging healthcare arena. Brown Urology faculty for the fellowship has grown over the years to include 3 Robotic Oncologists (Yale, MSKCC, Mayo Clinic) and 3 Fellowship trained Endourologists (Wisconsin, Brown, Mt.Sinai). Recent developments have allowed for optimization in research support, including IRB, grant writing and national database access support. Interested candidates may choose from multiple tracks which include:

  1. Endourology/Robotic Track – 1year (approx. 300 robotic cases/year) (approx.: 75 PCNL/with access teaching, 250 URS) – Options include: 1 year Endourology, 1 year Robotics or a combination of the two.
  2. Integrated fellowship with Brown University’s School of Public Health. The 2-year position has a primary focus on training an academic minimally invasive urologist and creating a future leader in our field. The fellow will graduate with an accredited Endourology Fellowship certificate as well as a Master’s in Public Health or a Master’s in Clinical and Translational Research.
  3. Research Scholar Track – 1 year – Basic Science Research in Oncology or Endourology

Except for the research scholar track, the fellow is responsible for coverage of all minimally invasive urologic surgical procedures (percutaneous renal surgery, endourology, laparoscopy, and robotics) cases on his/her operating room days. The fellow may be asked to interact with our residents during certain cases, but as “teaching faculty.” The fellow will be have an appointment at the Alpert Medical School of Brown University.

The applicant will have completed an ACGME approved residency in urology and be eligible to sit for part I of the American Board of Urology qualifying examination. Urologists who have passed part I of the American Board of Urology Qualifying Examination and are licensed to practice medicine in the United States are also eligible for consideration in the program. International graduates from non-ACGME Urology programs may apply if they have ECFMG certification and passed USMLE Step 3. Additionally current Rhode Island State Law REQUIRES trainees in non-ACGME Programs to get a FULL RI LICENSE. In order to qualify for Full RI License (rather than a Limited License), trainees MUST HAVE HAD at least 2 years of US ACGME training

  1. SCOPE OF TRAINING Specialized training in minimally invasive urologic surgery will cover all aspects of endourologic surgery at the Rhode Island and Miriam Hospitals – Including: minimally invasive urologic surgery of the lower and upper tracts, image guided therapy for urolithiasis and tumors, MIS BPH procedures, and various laparoscopic and robotic procedures. The subspecialty training in minimally invasive urologic surgery will provide experience of sufficient level for the trainee to acquire advanced skills in each of these areas. The fellow will work with current residents in the Brown University Urology program. The fellow may staff cases deemed appropriate by the program director as a junior faculty for current Brown urology residents. The fellow may develop a focus in a particular area of minimally invasive/endourological surgery during their year. EDUCATION PROGRAM The clinical status of a fellow is that of a junior staff member. The fellow will have graded surgical responsibility and will be responsible progressively for more and more of the operative care of patients. The fellow will also be responsible for the preparation of teaching conferences for residents and staff. In addition, the fellow will have a teaching role in clinical care and serve as a research mentor. The fellow will be required to participate in clinical research but can direct his/her efforts in areas of interest. There are multiple opportunities for interdisciplinary collaboration with other clinical departments and professional schools within Brown University.

This is a 1 year intensive high-volume fellowship in robotic and laparoscopic urologic surgery. Emphasis is heavily on oncologic and also on reconstructive cases. The program is geared towards a certificate in the above disciplines. At the end of the fellowship it is expected that the fellow will be proficient in robotic prostatectomy, robotic partial nephrectomy and robotic radical cystectomy.

The objective is to provide a mentored, supervised fellowship program with ample autonomy concomitantly. There are research opportunities available through extensive databases in robotic prostatectomy and minimally invasive partial nephrectomy and other oncologic databases for clinical and outcomes research.

The fellow will have protected time one day a week for clinical research endeavors. The fellow will have access to an active simulation facility at Einstein plus the Montefiore Institute of minimally Invasive Surgery with full animal lab capabilities. In addition, two robotic simulators are at the fellow's disposal.
