This is a 1 year intensive high-volume fellowship in robotic and laparoscopic urologic surgery. Emphasis is heavily on oncologic and also on reconstructive cases. The program is geared towards a certificate in the above disciplines. At the end of the fellowship it is expected that the fellow will be proficient in robotic prostatectomy, robotic partial nephrectomy and robotic radical cystectomy. The objective is to provide a mentored, supervised fellowship program with ample autonomy concomitantly. There are research opportunities available through extensive databases in robotic prostatectomy and minimally invasive partial nephrectomy and other oncologic databases for clinical and outcomes research. The fellow will have protected time one day a week for clinical research endeavors. The fellow will have access to an active simulation facility at Einstein plus the Montefiore Institute of minimally Invasive Surgery with full animal lab capabilities. In addition, two robotic simulators are at the fellows disposal.

This one-year fellowship offers a comprehensive training environment that provides experience with all aspects of endoscopic, laparoscopic, and robotic surgery at Yale-New Haven Hospital – the 4th largest medical center in the United States.

We have a large volume of stone disease, oncology, reconstructive, and BPH operations including HoLEP. There are five clinically dedicated da Vinci Xi robots and one single port da Vinci system. Our simlab has a dedicated research and training da Vinci robot and a da Vinci simulator.

Clinical fellows typically perform approximately 250 ureteroscopy, 80-100 percutaneous, 30 ESWL, 40-50 lap/robotic, and 100 HoLEP procedures over the year. Fellows gradate practice-ready to confidently perform challenging endoscopic and minimally invasive procedures.

Our fellows participate in the Yale multidisciplinary Stone Clinic, working side-by-side with a dedicated urologist, nephrologist and nutritionist.

We have robust surgical and metabolic datasets maintained by a dedicated post graduate fellow. Additional resources include experts in outcomes and large dataset research.

In addition to clinical and research responsibilities, the fellow is expected to embrace educational and teaching opportunities within the department such as attending conferences, giving lectures, and mentoring the resident staff.

While the fellow will have attending (operative and admitting) privileges, they will take no more call than any other staff attending urologist.

The one year Fellowship in Endourology, Laparoscopy, and Robotic surgery provides a very comprehensive training environment in all areas of minimally invasive procedures. We are a high volume robotic and laparoscopic center. Our program was one of the first programs adopting the laparoscopic radical prostatectomy in 2000. We have a very successful laparoscopic program which includes: donor nephrectomy (15 years), radical nephrectomy, partial nephrectomy, adrenalectomy, and cryoablation. We were involved in the FDA trials with the Zeus Robotic platform and early adopters with robotics in general. We have a successful Davinci program including: radical prostatectomy, partial nephrectomy, sacrocolpopexy , and pyeloplasty.

Our program is located in the heart of the stone belt and as such the fellow has exposure to a variety of simple and complex stone cases. In addition to having access to the latest ureteroscopic techniques, the fellow will learn how to perform percutaneous renal access. We perform our own metabolic evaluations and the fellow will learn the latest in medical management of stone disease.

Our fellows have been very productive academically. In the last 8 years, our program has placed first, second, third, and fourth in the Endourology Fellowship Essay Contest. We have a large research department supporting 50 clinical trials. We have a state of the art outcome database. Some of this research has been cited in USA Today and Reuters in addition to being accepted for publication in the Journals of Urology and Endourology. We have a nationally recognized Proteonomics lab with numerous projects available for fellow participation.

Finally, our program has nine fellowship trained urological oncologists and endourologists. Our fellows are part of the faculty and run a half day clinic once a week. Additionally, they have limited call responsibilities (on average one or two weekdays per month and 5-6 weekends) during their one year fellowship.

Job Information

The Endourology Program at the University of Wisconsin-Madison is an Endourology Society accredited fellowship. The fellowship program was established by Dr. Stephen Nakada in 2002.

Currently a one-year program, this fellowship has both a clinical and academic focus.

Clinical

Dr. Stephen Nakada, and Dr. Margaret Knoedler are fellowship co-directors. Kris Penniston, PhD, Dan Gralnek, MD, Ali Antar, MD, Emily Serrell, MD, and Allan Jhagroo, MD are the core fellowship faculty. Faculty in interventional radiology include Dr. Mark Kleedehn, Dr. Prasad Dalvie, and Dr. Jason Pinchot.

During the course of their fellowship, our fellows gain broad experience in clinical endourology, including complex ureteroscopy, percutaneous nephrolithotomy, and HoLEP.

Fellows will master the medical management of stone disease through participation in our multi-disciplinary stone clinic, under the mentorship of Kris Penniston, PhD, RDN, and nephrologist Dr. Allan Jhagroo. Academic pursuits in the fellowship, including fellowship essay, are led by Drs. Nakada, Knoedler, and Penniston.

Research

All endourology fellows have dedicated research time and a dedicated research space, including access to a PhD. Fellows are expected to complete a basic science project, as well as all of the appropriate Endourology Society fellowship requirements. Fellows also have access to a brand new, state-of-the-art Simulation Center to develop their skills and for potential investigations. A fully equipped animal laboratory is also available.

Fellowship in Endourology, Urologic Laparoscopy and Robotic-assisted surgery

Description of Training
The University of Toronto Endourology, Urologic Laparoscopy, and Robotic-assisted surgery fellowship is a comprehensive 2-year program that provides clinical and research training in both endourology and minimally-invasive urologic surgery. As well as training in all aspects of contemporary endourology, fellows will be involved with the medical management of stone disease through the Provincial Kidney Stone Prevention Clinic, and will gain experience with advanced laparoscopic and robotic assisted laparoscopic procedures. These include nephrectomy, donor nephrectomy, nephroureterectomy, adrenalectomy, and robotic assisted radical prostatectomy, partial nephrectomy and pyeloplasty. We serve as a provincial tertiary referral centre for complex stone disease, laparoscopic surgery and robotic-assisted surgery. Our centre is also one of only 3 shock wave lithotripsy units in the province of Ontario.

The Fellowship will consist of 2 years, with one year primarily clinically-oriented, and a second year primarily research-oriented.

Duration of Training
Fellows are required to have completed a recognized 5 or 6 year Urology Residency with the appropriate certification.

Laboratory Facilities and Research Supervision
Clinical databases are available to allow prospective outcome studies on 2,500-2,800 shockwave patients per year, as well as all patients treated endourologically, laparoscopically and robotically. Both basic science bench-space and a fully equipped animal vivarium are available. A Ph.D. is available to supervise specific projects, in areas ranging from biomaterials, infection, and metabolic stone disease. Supervision is also available for those wishing to pursue an M.Sc. in clinical epidemiology, and fellows are expected to participate in ongoing clinical trials, as well as in the development of new trials. Fellows are expected to submit abstracts to annual meetings and prepare manuscripts for publication.

Clinical Responsibilities
The Fellow will participate in a wide range of endourologic, laparoscopic and robotic assisted surgical procedures and round on these patients, with the senior resident, in the post-operative period. He or she will be involved with both the surgical and medical assessment of patients through the clinics at The St. Michael's Hospital Kidney Stone Centre. The Fellow will not be expected to take night or weekend call except for unusual circumstances. Fellows are invited to participate in renal transplantation if they so desire.
