Loyola University Medical Center Endourology Fellowship

Overview:

This is an intensive one year fellowship covering all facets of Endourology.  The fellowship involves comprehensive training in all aspects of minimally invasive urology. The fellow is integrally involved in the operating room with all robotic, laparoscopic, percutaneous, ureteroscopic  and ESWL procedures. The fellow is expected to supervise and educate residents in the operating room. The fellow has the opportunity to participate in surgical cases at three different hospital settings: our academic hospital (LUMC), the VA (Hines VA) and a private hospital (Gottlieb Memorial) where many of robotic faculty operative with the da Vinci Xi surgical system.  There are also research opportunities available to the fellow including many established clinical research databases. There is an expectation to produce two clinical manuscript during the fellowship year.

Applicants must be eligible for an unrestricted Illinois medical license and must have completed a urologic residency.

The Urology and Nephrology Center (UNC) at Mansoura University in Egypt is an internationally renowned urologic institute. With 250 beds totally devoted to urology, UNC surgeons perform an enormous number of different endourologic procedures in one of the busiest programs in the world. Our strengths in different realms of endourology including minimally invasive surgery, complex stone management and Laser prostate surgeries put us in a unique position to efficiently and effectively train our fellows.

Program at a glance:

The Endourology fellowship program at UNC is an Endourology Society certified training program.

The first year

During the first six months of the first year, the fellow will actively participate in basic science, laboratory and clinical research.  A fully equipped animal research laboratory is available at UNC. The fellow is asked to identify the research topics of his/her interest in addition to the proposal of research that will suggested be his/her faculty supervisors.

During of the 2nd six months the fellow will spend one day a week in the operating room (OR) while the research program will continue during the rest of the week. In the OR the fellow will assist in PCNL, laparoscopic procedures, and retrograde ureterorenoscopy cases. He will actively participate and perform rigid ureteroscopy particularly for cases with pelvic ureter pathology.

The second year

During this year, the fellow will be actively involved in the clinical practice and activity keeping one day per week for research work. The fellow will be able to actively participate to a minimum of 60 retrograde ureteroscopic and/ or ureterorenoscopic cases, 60 percutaneous renal cases, 100 laparoscopic procedures, 20 endoscopic surgical procedures for BPH and 10 ESWL.  By the end of the 2 years, the fellow should do 20% of these cases by himself or under supervision of the faculty members. .All the cases will be recorded in log book of the fellow and revised by the director of the course.

For stone management, the fellow will gain experience in shockwave lithotripsy, ureteroscopy both rigid and flexible, and percutaneous nephrolithotomy. We have dedicated two shockwave lithotripsy suites with latest technology.  A dedicated endourology suite is also present in our operating theatre. Percutaneous access is only performed by urologists in our institute and the fellow is expected to master it by the end of his fellowship.

Laparoscopic activity will start with dry lab training in the animal house for one month. Clinical training includes both ablative and reconstructive procedures. By the end of the training the fellow will master the ablative laparoscopic procedures particularly of the upper urinary tract and basic of the reconstructive procedures. Laparoscopic radical prostatectomy procedures are not widely available  because of the rarity of such pathology in our locality. The candidate will actively participate in advanced laparo-endoscopic single site surgery (LESS) and advanced reconstructive procedures. The state-of-the-art laparoscopy training facility is available which the fellow is expected to use to enhance his laparoscopic skills.

Recent modalities for management of BPH are frequently used and performed in our institute. The fellow will actively participate in various techniques including, Transurethral resection of the prostate using saline (TURiS), photoselective vaporization (PVP) and Holmium Laser enucleation of the prostate (HoLEP). Both Holmium VersaPulse 100 Watt and Greenlight XPS Laser machines are available. A unique dedicated prostate clinic is also available for evaluation and management of prostate-related urinary symptoms.

In fact, the only limits for the fellow’s productivity during the fellowship is his range of creative capabilities and his clinical progress. Upon completion of the fellowship program the fellow will be able to meet the requirements for certification by the Endourological society. The fellow is expected to maintain an accurate and comprehensive operative log that will be reviewed with the program directors on routine basis.

1 Year Advanced Clinical Training Certificate in Endourology/Stone Surgery

This is a 1 year fellowship, which focuses on upper tract urinary stones and TCC tumors.  

Fellows will perform a high volume of complex semirigid and flexible ureteroscopies and supine PCNL, including their own access under

fluoro and ultrasound control.

Job Information

Overview:  The Endourology Program at the University of Wisconsin-Madison is an Endourology Society accredited fellowship, and has been since inception.  The fellowship program was started by Dr. Nakada in 2002. In 2018 the University of Wisconsin Program in Endourology will go from a 2-year fellowship to a one-year certificate program in Endourology and Stone disease.

The fellowship is based in Madison, WI at the UW Hospital and Clinics.  Fellows are hired as clinical instructors, and participate surgically at all levels, including PCNL (access, stone removal and drainage), URS, SWL, endoscopic ablation and incisions, HOLEP and alternative BPH techniques. Additionally percutaneous endoscopic biliary lithotripsy, renal ablation and robotics are available if the fellow is so inclined.

Important research opportunities exist, specifically in quality of life work, database analysis, RCTs, and basic research.  There is a dedicated PhD and laboratory space available to the fellow.

The fellow also participates in the Metabolic Stone Clinic and develops the fundamental skills and knowledge required to become an expert in the medical management of all stone patients. This experience emphasizes the benefits of multidisciplinary clinical care and features exposure to shared medical appointments. The fellow will function as part of the team that includes a nephrologist (Dr. Allen Jhagroo) and nutrition specialist (Dr. Kristina Pennistion).

The fellow will also learn the rationale for medical nutrition therapy and how it is practiced and applied. Specifically, the fellow will:

-understand dietary risk factors and how they contribute to stones

-how to identify dietary risk factors and to screen

-make appropriate dietary recommendations to patients based on their individual dietary and stone risk factors.

Fellows participate in the weekly stone clinic conference, including delivering presentations as assigned (average of about 12 per year).  We send the fellow to the WCE each year they are in the program.

Description of Training The University of Toronto Endourology, Urologic Laparoscopy, and Robotic-assisted surgery fellowship is a comprehensive 2-year program that provides clinical and research training in both endourology and minimally-invasive urologic surgery. The training program includes two training sites in downtown Toronto – St Michael’s Hospital (http://www.stmichaelshospital.com/) and Toronto General Hospital (https://www.uhn.ca/OurHospitals/TGH)

The Fellowship will consist of one year primarily focused on stone disease/endourology and the other year focused primarily on advanced laparoscopy and robotic-assisted surgery.  Clinical research occurs longitudinally throughout the fellowship. Involvement in the kidney transplant program will also occur longitudinally over the 2 year program.

Surgical training during the endourology-focused year will involve all procedures in contemporary endourology, including the ability to get your own percutaneous access. Upon completion of training, fellows will be comfortable performing stone-related surgery in standard patients as well as complex patients (eg renal transplants, urinary diversions, etc). Training during the MIS year will involve laparoscopic procedures such as simple and radical nephrectomy, donor nephrectomy, nephroureterectomy, adrenalectomy. Robotic-assisted procedures include partial nephrectomy (trans and retroperitoneal), radical prostatectomy, pyeloplasty.

In addition to surgical training, fellows will be working in the St Michael’s Hospital Kidney Stone Centre, a regionalized referral centre for shock wave lithotripsy (one of only 3 in the province of Ontario). As such, fellows will garner extensive experience in the medical management of stone disease and SWL. 

Training Requirements Fellows are required to have completed a recognized 5 or 6 year Urology Residency with appropriate certification.

 

Laboratory Facilities and Research Supervision Research requirements span across the 2 year training program and can be fellow-initiated projects as well as faculty-initiated studies. Clinical databases are available to allow prospective outcome studies on 2,500-2,800 shockwave patients per year, as well as all patients treated endourologically, laparoscopically and robotically. Opportunities to conduct surgical education research, surgical innovation and technology assessment research, kidney transplant studies, and clinical epidemiologic research are also available. A research coordinator is available to support all fellow projects, including REB/IRB applications. Fellows are expected to submit abstracts to annual meetings and prepare manuscripts for publication. Fellows interested in pursuing a concurrent MSc or PhD degree are encouraged to discuss options with the fellowship director.

Clinical Responsibilities The Fellow will participate in a wide range of endourologic, laparoscopic and robotic-assisted surgical procedures as outlined above. The fellow, along with the urology resident team, will be responsible for managing and rounding on the surgical patients in the peri-operative period. The fellow will also be responsible for ambulatory clinic duties within the SMH Kidney Stone Centre. Fellows will not be expected to take overnight or weekend general urology call, except for rare extenuating circumstances. Fellows will be expected to participate in both the living donor (both sites) and deceased donor renal transplant programs (at SMH site only) during the course of fellowship.
