Endourological Society – King Abdulaziz Medical City

King Abdulaziz Medical City in Riyadh is an international award winning comprehensive medical center facility with an outstanding reputation as one of the nation’s largest health care organizations that is committed to clinical excellence and casting the light on patients and families as an outmost target.  It has a dedicated team of physicians, nurses, and clinicians that work to provide all patients with the quality care in an atmosphere of compassion. The hospital includes 2,500 inpatient beds, 100 ICU beds, and 25 operating room suites.

The Division of Urology is a national key discipline in Riyadh with an estimated annual DCOR and main OR volume of 4000 procedures including all varieties of urological endoscopic and laparoscopic/robotic surgeries. In addition, the division of Urology possesses 5 outpatient clinics and cystoscope room and a urodynamic examination suite. The Urology team members have a strong urological talent force as some of them hold prestigious academic positions in different academic groups.

The Endourology and Minimally Invasive Fellowship Programs at King Abdulaziz Medical City offers two distinct fellowship streams. Candidates may apply for one of these two programs.

Description of Program

This is a two-year fellowship program in advanced Endourology and stone disease.

The first year:

During the first six months, the fellow will actively participate in basic science, laboratory and clinical research.  The fellow is asked to identify the research topics of his/her interest in addition to the proposal of research that will be suggested by his/her faculty supervisors.

During the second six months, the fellow will spend one day a week in the OR while the research program will continue during the rest of the week. In the OR the fellow will assist in PCNL, RIRS, TURP, et…...

The second year:

During this year, the fellow will be actively involved in the clinical practice and activity keeping one day per week for research work. The fellow will be able to actively participate to a minimum of 60 PCNL cases, 120 URS and/ or RIRS cases, 60 endoscopic surgical procedures for BPH. All cases will be recorded in logbook of the fellow and revised by the fellowship faculty.

Upon completion of the fellowship program the fellow will be able to meet the requirements for certification by the Endourological society. The fellow is expected to maintain an accurate and comprehensive operative log that will be reviewed with the program directors on routine basis.

2)  2-YEAR COMBINED ENDO/STONE, LAP/ROBOTIC FELLOWSHIP focusing on Endourology and stone disease, and laparoscopic and robotic oncologic and reconstructive procedures. Description of Program

This is a two-year fellowship program involving both research and clinical experience in endourologic and laparoscopic/robotic interventions. Trainees can receive step-by-step training in a systematic and standardized teaching and training approach. Surgery types include laparoscopic or da Vinci robot-assisted laparoscopic radical prostatectomy, radical cystectomy (ileal bladder replacement), and kidney surgery (resection of renal cancer, partial nephrectomy, renal cancer with vena cava tumor thrombectomy, etc..), and robot-assisted laparoscopic urinary tract reconstruction. Furthermore, all endourology procedures are covered in terms of PCNL, RIRS, TURPS, HoLEP, TURBT, etc…..

Research Opportunity

Selection of fellow:


A. Applicants from the United States must have completed an accredited Urology Residency program.

B. Applicants must be board-eligible or board-certified at the commencement of the fellowship year. A current New York medical license must be obtained. Previous evidence of training and clinical experience in basic laparoscopic skills must be documented.

C. Letters of recommendation will be required from the individual's Program Director and two other individuals familiar with the applicant's training and performance.

D. Non-US applicants will be considered for the position if they can obtain a license to practice medicine in New York and have completed a urology residency. The applicants' visa status must be current and allow for clinical activity.

Description of Training:

The Endourology Fellowship Program at Stony Brook Medicine offers trainees a one-year training experience in endourology and nephrolithiasis.  The focus of the Fellowship is to provide applicants the opportunity to develop their clinical, surgical, metabolic and research skills required to be an expert in treating patients with stone disease.

Our faculty consists of 3 attendings who specialize in stone disease and minimally invasive surgery. Fellows will have the opportunity to work with David Schulsinger, MD and Gauthami Churukanti, MD at Stony Brook Medicine and , John Fitzgerald, (North Port VA) in clinical, surgical, metabolic and research capacities.  The Fellowship is structed to include a combination of clinical/surgical (90%) and Research (10%) training over the 1-year training period under the guidance of the faculty and with the resources of Stony Brook Medicine.  With a large volume of endourologic procedures at Stony Brook, fellows will receive a comprehensive experience in all aspects of Endourology.

Clinical/Surgical Training

Endourology Fellows will work with Drs. David Schulsinger, Gauthami Churakanti and John Fitzgerald to enhance their endourologic surgical skills.  The goal for the Fellow will be to master all aspects of management of stone disease.  This will include clinical and surgical management of stone patients with complex metabolic disturbances and anatomical anomalies.  Surgically, Fellows will develop advanced skills in PCNL and URS.  Additional training will include percutaneous access with Dr. Ferretti for other urologic conditions.  Fellows will utilize the latest technology of lasers and instrumentation for surgically managing patients with stone disease.  In addition, fellows will develop their endoscopic surgical skills for managing patients with oncological diseases with controllable energy forms, including cryoablation for treatment of prostate and renal tumors.  In addition, Fellows will treat patients for benign diseases with Greenlight laser (GLL) of the prostate and our developing program in Holmium laser ablation (HoLEP).

As an integral part of the Endourology training program for our residents, the Fellow will aid in teaching and mentoring basic and advanced ureteroscopic, percutaneous and endoscopic procedures.  The fellow will contribute to Urology Grand Rounds, Journal Clubs and Research Conferences.  The Fellow will be employed as a junior faculty member.

By the completion of Endourology program, the fellow will have gained the resources to independently provide expert level endourologic care and clinical management to their urology patients.

 1. OVERVIEW

 Endourology at Hamad Medical Corporation is a Fellowship program imparting training to the clinical fellows in the field of Endourology for last 10 years. It is a well- structured program which provide higher level of training with well- developed curriculum and schedule.

The treatment of urinary tract stones forms a major part of the workload of the urology   department in Hamad Medical Corporation  

The high incidence of stones in Qatar, the different nature of the stone disease and the rapid change in the means of stone removal necessitate the Urologist to have special training to acquire the necessary skills in various endourologic techniques and ESWL. 

Urology Department at Hamad Medical Corporation has the state-of-the-art facilities for the diagnosis and treatment of Urolithiasis.

 

2.    GENERAL OBJECTIVES

Fellowship Program aims to train selected Urologist in the field of Endourology.

The candidate is expected to gain special skills in the following:

  1. Various aspects of diagnostic and operative endourology using state of the art set up.
  2. Percutaneous Renal Surgery
  3. Retrograde Intrarenal
  4. Extra Corporeal Shockwave
  5. Interventional
  6. Foundation of an academic career including teaching skills, grant writing skills, manuscript preparation, research methodology and critical appraisal of
  7. Contribute in designing guidelines for the management of Urolithiasis in Qatar
  8. Set up an up to date urolithiasis

 

3. PROGRAM PERSONNEL

a.    Program Director:

A Program Director is a full-time faculty member. He will monitor the program contents, supervise the training process and ensure the high quality of the program. The program director of the Endourology fellowship at Hamad Medical Corporation is Dr. Ammar Al Ani.

b.    Teaching Staff:

Fellows will have the chance to learn from:

  1. Dedicated faculty specialized in Endourology will supervise fellows in their rotations, providing in-depth knowledge and skills in this field of Endourology.
  2. Regular visits of distinguished guests from outstanding institutes who will shed the light on various state of the art treatment modalities and surgical techniques.

 

4.    PROGRAM FACILITIES 

  1. Operating theatre: access to the operating theatre is granted on scheduled basis during which the fellow will perform various endourologic procedures according to the logbook requirement.
  1. Stone clinic: This is a specialized clinic that provides service for patients with recurrent stone disease. The fellow will gain in-depth experience in the metabolic evaluation and dietary advice to recurrent stone formers.
  • iii. ESWL Suite: This unit is well-equipped with two advanced machines, performing around 12 cases per day. The fellow will gain in-depth experience about the physics of shock wave, response to treatment and various maneuvers in stone breaking.
  1. Continuity Clinics: Fellows are expected to attend clinics under indirect supervision where he/she will manage patients with different urologic conditions.

5.    PROGRAM OUTLINE

The duration and the scope of training will encompass a total of 2 years (24 months).

Clinical Training:

In 2004, Hackensack University Medical Center founded the first fellowship training program in New Jersey accredited by the Endourological Society. As one of the earliest established fellowships in the country, its outstanding graduates have become leaders at their respective institutions.

The Advanced Clinical Training Program is a 1-year combined robotic/laparoscopic and endourology training program that provides highly specialized and comprehensive training with a strong emphasis on urologic oncology and reconstructive procedures. The focus of this program is to provide the trainee with an armamentarium of robotic, laparoscopic, and endourologic surgical skills through a training program that emphasizes hands-on involvement in operative cases. In addition, clinical research will complement the surgical experience.

The majority of the clinical training is in advanced surgical techniques in which the fellow will perform hundreds of surgical cases spanning routine as well as highly complex surgeries. The fellow is trained in a wide variety of procedures including:

The fellow actively participates in all aspects of clinical care such as diagnosing and evaluating patients, performing surgery, and postoperative care. Fellows receive a staff appointment and attending privileges at Hackensack University Medical Center. As such, an integral part of the training involves teaching and supervising urology residents and medical students, participating in weekly grand rounds, visiting professor conferences, and teaching postgraduate minimally invasive training courses.

The subspecialty training offered in this fellowship provides an exceptional experience that will prepare the graduate to be a future leader in minimally invasive urologic surgery. Fellows that have specific areas of interest are encouraged to personalize their training in order to enhance their fellowship. In addition to the comprehensive curriculum, the autonomy and hands-on surgical experience will provide the necessary skills to pursue a successful career in academics or specialized community practice settings.

Research:

A portion of the fellow’s time will include research. One of our department’s clinical research efforts involves developing novel surgical techniques and evaluating new technologies. An additional research focus is in the area of outcomes research using validated mechanisms and evidence-based medicine. We currently have robust prospective and retrospective databases with thousands of patients, as well as statistician support for analyses. Research endeavors will enrich the experience of the fellow and provide an opportunity for scholarly pursuit.

The David and Alice Jurist Institute for Research houses and consolidates the medical center’s ongoing research activities into one location. The top floor of this modern facility houses the state-of-the-art minimally-invasive and robotic surgery training and research center containing multiple da Vinci Surgical Systems. This center is fully equipped for animate and inanimate training and the fellow will have access for conducting research studies.

In addition, Hackensack University Medical Center partners with several local institutions and is involved in ongoing research projects. The fellow can be involved in current projects as well as new endeavors with investigators at the Hackensack Meridian Health Center for Discovery and Innovation and Hackensack Meridian School of Medicine.

The fellow will publish and present the results of their research work at national and international meetings.

The goal of the fellowship is to train endourologists to improve the care of patients with complex stone disease and BPH. During the fellowship year, trainees will master Holmium Laser Enucleation of the Prostate (HoLEP), which is a state of the art minimally invasive procedure.

The program director Amy Krambeck, MD teaches the work up and medical management of stone disease, including significant surgical experience with percutaneous renal access, advanced percutaneous nephrolithotomy techniques, advanced ureteroscopy, and HoLEP.

The fellowship includes 20% research under the guidance of the faculty and with the resources at Northwestern University Feinberg School of Medicine.

By program completion, fellows will have the expertise to independently provide expert-level endourological care to patients.

Fellows will join the urology housestaff at weekly didactic conferences, including urology grand rounds, pre-operative conference, and a fellowship-level endourology conference.
