The UW Endourology and Minimally Invasive Surgery Section offers an opportunity for a two-year fellowship (Endourology) with a strong focus on endourology/stones.  Fellows in our program will experience high case volumes, complex case mixes and a vast array of research opportunities. UW was awarded a U01 from the NIH to become one of four clinical centers forming the Urinary Stone Disease Research Network (USDRN). Through a P01 award, several ultrasound-related studies are underway and planned for the coming years.

The 2 year fellowship is focused on clinical endourology and kidney stones; in addition, the fellow will participate in advanced laparoscopic/robotic surgery weekly during the clinical year.  The fellow will attend our Comprehensive Multidisciplinary Stone Clinic and weekly Endourology conference. Throughout the duration of training, the fellow will primarily be mentored by three Endourology Faculty: Program Director Jonathan Harper M.D., Mathew Sorensen M.D. and Robert Sweet M.D.

A variety of research opportunities are available during the first year that can continue into the second (clinical) year.  Drs. Harper and Sorensen have an active collaboration with the UW Applied Physics Laboratory and Center for Industrial and Medical Ultrasound.  The fellow will have the opportunity to work with research faculty Michael Bailey PhD, Adam Maxwell PhD and others.  The group continues to investigate ways to improve ultrasound for the detection and treatment of kidney stones, such as ultrasonic propulsion and burst wave lithotripsy.  Additional opportunities for research lie within the Center for Research in Education and Simulation Technology, lead by Dr. Sweet.

The University of the Minnesota was the Birthplace of Endourology. In the early 1980’s, Drs. Clayman, Smith, Lange, Reddy, Amplatz and Castenada-Zuniga joined forces to develop the field of minimally invasive urological surgery and train urologists in this new art. We are proud of this heritage and you will be also. The Goals of our Fellowship program are:

Description of Program
The structure and of the fellowship will be tailored to the fellow applicant with the content determined by the fellow’s prior experience in endourology, laparoscopy, robotics and urologic research as well as the fellow’s career objectives related to clinical and research focus. The fellow may select to study from one or more of the areas of endourology, metabolic stone disease, laparoscopy, robotics, thermal therapy, urologic oncology, virtual reality applications in medicine and surgical simulation. Research opportunities in medical device design and evaluation, computer simulation, thermal therapy, renal physiology, and multiple clinical studies are available.

 

BENCH SPACE AND ANIMAL LABORATORY FACILITIES
Both bench and animal facility space is available for research use. The experimental surgical center (ESS) at the University of Minnesota have ten laparoscopic animal surgical suites, while the VAHCS Minneapolis has an additional three suites. Both sites have fluoroscopic capability. A da Vinci robotic system is available for training and research in the ESS. In addition we have over 5000 sq feet of space for the development and implementation of skills training and computer simulation.

ON CAMPUS HOUSING
While no on campus housing is available, there are numerous housing options from apartments and condominiums to homes for rent within walking distance of the campus. The fellow may be involved in research studies that require after-hours responsibilities. The fellows will be included in the call rotation.

Dr. Sweet completed an AFUD Scholarship in Surgical Simulation at the University of Washington under the direction of Dr. Richard Satava. Dr. Sweet has a clinical focus in Endourology and Robotics and his research focuses on simulation development, robotics and applications of virtual reality in medicine.

The history of the Endourology fellowship at the University of Minnesota is long and rich - training leaders in the field such as Howard Winfield, Raymond Leveillee and Joel Teichman. Our recent graduates are:

Minimally Invasive Urologic Surgery (Endourology)

The University of Minnesota, Department of Urology offers a 1-year clinical fellowship in minimally invasive urologic and endourologic surgery. This fellowship program is led by Dr. Michael Borofsky.

Our fellowship program emphasizes minimally invasive urologic surgery with an emphasis on kidney stone disease as well as the management of benign prostatic hypertrophy. The fellow will participate in providing comprehensive kidney stone management including gaining a fundamental understanding and being able to provide treatment for metabolic stone disease, outpatient clinical stone care, and endoscopic surgical treatment of all stone types. Endourologic techniques including retrograde ureteroscopic renal surgery as well as percutaneous renal surgery including ultrasound and fluoroscopic guided percutaneous access.

The fellow will also participate in the outpatient workup and management of benign prostatic hypertrophy (BPH). There will be an opportunity to gain expertise in complex BPH surgery with an emphasis on holmium laser enucleation of the prostate (HoLEP).

The fellow has hospital staff privileges and takes call on the same schedule as the faculty (approximately 1 week in 8). They will be expected to participate in all aspects of patient care and function independently in surgical cases. They will be adequately supervised at all times for surgical cases and are primarily responsible for the perioperative management of all surgical and hospitalized patients.

He or she will primarily be based at the University where they will have a clinic of his/her own for 0.5 to 1 day a week. While there, the fellow will participate in the operating room with endourology faculty. The fellow will also have opportunities to spend time in the operating room at our affiliated Kidney Stone Institute in St. Paul (formerly HealthEast hospital system). The fellow gains graduated responsibility throughout the year in both the clinic and the operating room.

DURATION

The clinical portion of the experience is 12 months in duration. The fellow gains graduated responsibility throughout the year in both the clinic and the operating room.

CURRICULUM & RESEARCH

A structured curriculum, weekly conferences, and close interaction with urology residents are provided. One day per week is protected as research time. A research meeting is held for 1-2 hours each week on the research day where research group members review progress on our clinical and/or outcomes research. The fellow is expected, with the help of residents and our team, to complete multiple research projects in the course of the year.

CASE MIX

After completing a one-year fellowship, the graduate should be proficient in the following: Retrograde ureteroscopic stone treatment, percutaneous nephrolithotomy, and holmium laser enucleation of the prostate. Typical case volumes across the medical system provided by participating faculty for the aforementioned procedures in a calendar year are as follows:

-Retrograde Ureteroscopic Stone Treatment. 650 cases

-Percutaneous Nephrolithotomy 125 cases

-Holmium Laser Enucleation of the Prostate 100 cases

EVALUATION

To ensure the fellow gains proficiency and develops the corresponding technical skills, supervising faculty members monitor performance throughout this program. Supervising faculty completes competency-based electronic evaluations after each quarter. The fellow will then meet with the program director to review these evaluations on a quarterly basis or more frequently if necessary. In addition, they will have the opportunity to evaluate the faculty to ensure educational goals are being met.

Admission is contingent upon completion of an ACGME-accredited urology training program.
The applicant must be a US citizen.
The applicant must have three letters of support
The applicant must have passed all 3 steps of USMLE before the match date and be eligible for a Minnesota State medical license prior to starting fellowship.

Department of Urology:

The Department of Urology at Montefiore has 16 full time urologists and 2 PhD faculty, representing all subspecialties within the field. The urology department faculty are enthusiastic about teaching, and continue to develop interactive and engaging ways to teach with our trainees. Our 5-year ACGME accredited residency program grew from 2 residents/year to 3 residents/year in 2022. We perform a high annual surgical volume of endourological (500+ ureteroscopy, 110+ percutaneous nephrolithotomy) and robotic/laparoscopic (200+) operations.

The urology department faculty are enthusiastic about teaching, and continue to develop more interactive and engaging ways to teach our residents. Within the program, residents play a key role in caring for our patients and are important members of every care team. Endourology & Minimally Invasive Urological Surgery Fellowship:

The Montefiore Combined Endourology & Minimally Invasive Urological Surgery fellowship is a 1-year Endourology Society-accredited Certificate Program. We are excited to re-launch the fellowship for 2023. Our fellow will be fully integrated into the clinical, academic and educational activities of the urology department.

The training program is focused on advanced laparoscopy/robotic surgery as well as complex stone treatment. Our busy urban practice has a high volume that will expose the fellow to a wide breadth of surgical procedures. The program focus can be tailored to an individual fellows’ interests and career goals. At Montefiore Moses and Weiler locations, there are four daVinci Xi surgical systems with dual consoles. We routinely perform robotic partial nephrectomy, pyeloplasty, ureteral reconstructions, cystectomy, prostatectomy, and other complex procedures. PCNL and ureteroscopy procedures happen on a daily basis.

The fellow will function as a junior faculty member and will be appointed as a clinical instructor. They will supervise residents in the clinic and OR and book their own cases. They will share attending call with the other faculty (approximately 4 calls/month).

Annual salary is based on the PGY-6 Montefiore residency pay scale ($83,000 in 2022) with full fringe benefits.

Fellowship Overview

The University of Maryland – Chesapeake Urology Endourology Program is designed for urologists who wish to enhance their surgical skills and broaden their clinical experience in preparation for an academic career or as a prelude to entering practice as a urological sub-specialist. This is a one-year clinical fellowship at Chesapeake Urology and The University of Maryland that focuses on the medical and surgical management of stone disease under the leadership of renowned PCNL and stone disease specialist, Dr. Julio Davalos. The goal of the program is to enhance endourologic surgical skills and gain expertise in surgical management of complex stone cases as well as medical management of stone disease. Case volumes are expected to exceed 400 per year.

A particular emphasis is on percutaneous nephrolithotomy (PCNL) in the following areas: Outpatient same day discharge PCNL, Ambulatory (ASC) PCNL; multi-tract PCNL; Single Incision Multiple Access (SIMA) PCNL; tubeless PCNL; totally tubeless PCNL; and smaller tract PCNL including mini, uItra-mini, and micro-PCNL. Both prone and supine PCNLs are performed.

Renal access is specifically highlighted throughout the training. Anatomic guided, fluoroscopic, ultrasound and endoscopically visualized access will all be covered.

Additional areas covered:

  • Retrograde endoscopic surgery (Ureteropyeloscopy case volume 150).
  • Percutaneous and endoscopic management of upper tract urothelial cancer.
  • Endoscopic management of stricture disease.
  • Metabolic workup and management of urolithiasis, aimed at prevention.

Opportunities for robotic assisted laparoscopic (RAL) training in upper tract reconstruction and benign disease will also be offered and are optional, including RAL Pyeloplasty, Ureteroureterostomy, Ureteroneocystotomy, Renal Cyst Decortication, heminephrectomy, and nephrectomy.

How to Apply:

The applicant must be on track to complete an accredited urology residency-training program in the United States. The applicant must have completed residency and be board eligible upon the start date of the fellowship.

Applications are due by May 1st for the following year. Interviews for the position can be scheduled on an individual basis for candidates who wish to make a site visit and meet with the faculty in Baltimore, MD. In addition, interviews will be conducted at the annual AUA conference.
