Northwestern University Medical School Department of Urologic Surgery

Description of fellowship
The endourology fellowship at Northwestern University is a two year program. The clinical component and research component are done over two years with the fellow spending fifty percent of his time in the operating room, and fifty percent of his time doing basic science or clinical research. The clinical component primarily includes robotic and laparoscopic procedures for prostate cancer, bladder cancer, and kidney cancer, with a strong emphasis on: robotic prostatectomy, laparoscopic and robotic assisted partial nephrectomy and robotic cystectomy. Reconstructive cases are also done laparoscopically and robotically, and they primarily include pioplasty and ureteral reimplantation. The significant component of the fellow is the surgical experience which also pertains to endourology, with a strong emphasis on ureteroscopic and percutaneous management of stone disease and upper tract transitional cell carcinoma. A small component of the clinical work is done in the clinic.

From a research standpoint, Northwestern University has an exceptionally strong program in nanotechnology and molecular biology. Current projects include using nanotechnology to evaluate margin status and recurrence after robotic prostatectomy, as well as preservation of erectile dysfunction. We maintain a strong database of laparoscopic and endourology procedures, and the fellow is expected to participate in approximately ten clinical papers per year.

The fellow works primarily with Dr. Robert B. Nadler, Professor of Urology and head of the section of minimally invasive urologic surgery. The fellow also works closely with Dr. Kent Perry, Associate Professor of Urology to supplement his surgical experience. Overview of Program

Clinical

The fellowship program is a one or two-year postgraduate program designed to train urologists in Complex Stone Disease (PCNL) as well as advanced Laparoscopy and robotics. This program integrates broad exposure to stone disease, medical management of stone disease, urologic oncology, and the opportunity to take advantage of nearby Maria Ferreri Children's Hospital to learn pediatric endourology.

The expected breakdown of cases/time is roughly 70% endoluminal endourology (stones, reconstruction, upper tract urothelial carcinoma); 10% robotic/laparoscopic; 10% clinic; 10% research.

Complex Stone Disease:

The fellow can expect wide exposure to all manner of percutaneous procedures including, Supine PCNL, Prone PCNL, Standard PCNL, Mini-PCNL, Diverticular cases.  Renal Access will be taught using both Flouroscopic: Monoplanar, Biplanar, Triplanar, Bullseye, Endoscopic Combined Intrarenal Surgery (ECIRS), and Ultrasound Techniques.

Urologic Oncology and Robotic/Laparoscopic Surgery

 The fellow will can expect exposure to all manner of robotic urologic procedures including prostatectomy (radical and simple), nephrectomy (radical and partial) and cystectomy using the DaVinci Xi.

Research

The fellow is expected to participate in clinical research, focused on all types of endourology.  They will have access to an IRB approved endourology database focusing on clinical outcomes and are encouraged to develop their own research proposals and projects. In addition, basic science research labs are available, though not required, and the fellow may design his/her own experiments or join an ongoing experiment and work in collaboration with the Director of Research.

The facility contains dry labs and simulators for laparoscopic skills training as well as a vivarium for IRB approved research protocols.  The onsite basic science lab is staffed by a full time dedicated urologic researcher. There is also access to nearby industry sponsored surgical training facilities for laparoscopic and robotic skills training.  Westchester Medical Center and the adjoining are equipped with the latest state of the art surgical equipment (DaVinci Xi Robot, Thulium laser, Holmium laser).

The fellow will be working closely with 3 fulltime attending and several voluntary staff and will be an active participant in the operating room and will perform and assist in various endourologic, laparoscopic and robotic cases.

Originating in 2008, the Endourology Fellowship at Loma Linda University is designed to educate and train excellent clinicians and future academic leaders. There are two fellowship opportunities available, including Endourology/Stone Disease/BPH and combined Endourology/Minimally Invasive Surgery. Depending on the applicant’s interests, they can become proficient in all facets of minimally invasive urology. The training can include robotic, ureteroscopic, percutaneous, and minimally invasive treatment options for conditions requiring image-guided interventions. Loma Linda University's Endourology Fellowship provides fellows with a solid foundation in education and leadership, coupled with extensive research and clinical training. This training aims to equip fellows with the tools necessary for successful careers at major academic institutions and in high-volume private practice settings.

Leadership and Faculty

Under the guidance of the program director, Dr. D. Duane Baldwin, fellows benefit from LLU faculty members' extensive knowledge and expertise in endourology, treatment of BPH, and urologic oncology. Loma Linda University is located in the Stone Belt and is at the 99th percentile volume for complex kidney stone disease. The fellowship program is supported by an exceptional faculty of minimally invasive urologic oncology experts, including Dr. Herbert Ruckle (Chairman), Dr. Brian Hu, Dr. Martin Hofmann, Dr. Muhannad Alsyouf, and Dr. Antoin Douglawi. Fellows also work closely with Dr. David Benjamin in minimally invasive surgical treatment (MIST) of benign prostatic hyperplasia, including prostate enucleation. Further collaboration extends to Dr. Jason Smith, chief of interventional radiology, and Dr. Daniel Kwon, who provide additional expertise in complex image-guided renal access and cryoablation of small kidney tumors.

Clinical Training

Fellows undergoing clinical training will be exposed to comprehensive aspects of surgical and medical management for urinary stone disease. Fellows work closely and extensively with Dr. Baldwin, learning advanced fluoroscopy as well as ultrasound-guided PCNL, ureteroscopy, and image-guided renal cryoablation. They will also learn novel advanced techniques for ureteroscopy without guidewires or stents. Their training encompasses surgical endourologic techniques, and they achieve proficiency in endourology and robotic surgeries as prerequisites for fellowship completion. Upon graduation, fellows will demonstrate expertise in the optimal utilization of fluoroscopy and ultrasound to improve and simplify renal access techniques. They will also acquire advanced skills in ureteroscopy and advanced robotics. The typical fellow performs more than 300 ureteroscopies, 150 percutaneous nephrolithotomies (PCNL), and 100 robotic surgeries.

Research Training

Fellows are anticipated to actively engage in clinical and translational research pertaining to Endourology and Stone Disease. We are actively working with companies to develop the laser of the future and advance Direct In-Scope Suction ureteroscope technology. Fellows will integrate into a well-established research pipeline and participate in preparing manuscripts for publication, co-authoring book chapters, and presenting research findings at national and international meetings. It is expected that fellows attend the Annual Meeting of the AUA, the World Congress of Endourology, and the Annual Meeting of the Western Section of the AUA.  

Fellows will work with a dynamic team of full-time undergraduate research students, medical students, urology residents, international research fellows, and a dedicated research coordinator.  

Schedule

The fellowship program offers a structured weekly schedule to balance clinical and academic responsibilities:

  • Mondays and Tuesdays: Surgery days in the operating room
  • Wednesdays: Clinic duties for patient engagement
  • Thursdays: Alternating between operating room and clinic appointments
  • Fridays: Dedicated to academic activities, including research and lectures

This schedule ensures a well-rounded and immersive learning experience, equipping fellows with the skills and knowledge necessary for a successful career in endourology.

Candidate Expectations

Prospective candidates are expected to have successfully completed a urology residency. Experience in endoscopic techniques is important for applicants. A demonstrated inclination towards research and academics and a proven track record of publishing manuscripts is desirable. Candidates should possess or be eligible to obtain a medical license to practice in the State of California prior to starting the fellowship. International applicants must have an ECFMG certification to be considered.

The Brady Urological Institute of the Johns Hopkins Medical Institutions offers a highly specialized and comprehensive fellowship training program in Endourology and Minimally Invasive Urological Surgery. This program is unique in that fellows are exposed to all aspects of minimally invasive urology in high volume – including multi-port and single-port robotics, laparoscopy, HoLEP, PCNL (prone, supine, fluoroscopy-guided, ultrasound-guided, endoscopic combined intra-renal surgery), Ureteroscopy, ESWL, cryoablation, and HIFU.

The fellowship is a two-year program with integrated clinical and research components. It is based at four campuses of the Johns Hopkins Medical Institutions. Primary sites are Johns Hopkins Hospital and Johns Hopkins Bayview Medical Center, both located in Baltimore, Maryland. Additionally, the program extends to Howard County Medical Center in Columbia, MD, and Sibley Hospital in Washington DC.

At Hopkins, fellows will have the ability to leverage collaborations within the department and across the school. The Brady Urological Institute has several funded PhD researchers in basic sciences as well as its own URobotics engineering lab led by Dr. Dan Stoianovici. With the addition of Dr. Ahmed Ghazi, fellows have access to the Surgical Learning and Innovation Center of Excellence (SLICE) to use patient-specific hydrogel phantoms for surgical training and research. Fellows can also pursue individual interests across the Whiting School of Engineering or the Bloomberg School of Public Health.

The fellow is appointed as a faculty member with the title of Instructor in Urology; they have full hospital admitting and operative privileges, allowing the fellow to gain surgical autonomy and develop experience in resident education. The fellowship is structured to train individuals interested in pursuing a career in academia.

Other responsibilities of the fellow include managing their own patient clinic once per week and call coverage for approximately 4 weeks of the year. As a result, fellows are required to have a full state medical license. The fellows are not expected to attend faculty clinics nor manage EHR inboxes. The expectation of the fellow is to leverage the program to meet their future clinical and academic needs. Fellowship schedules can be adjusted depending on the interest of the fellow to focus in some areas more than others.
