Program Demographics 

 

The Program The Urology/Endourology Fellowship Program at Indiana University School of Medicine in conjunction with Methodist Hospital has been in existence since 1990 and credentialed by the Endourology Society. The fellowship program will be a one year experience with 2 positions available.  There will be an option for a 2 year fellowship if one is desired but must be requested prior to the match.  The clinical time during the fellowship will consist of 70-80% clinical with approximately 1 day of clinic, 3 days in the OR and 1 day of research.  

 If a two year fellowship is selected the fellow will be encouraged to pursue a Master’s Degree in Public Health, a Masters in Biostatistics through IUPUI free of charge.  Residents completing an ACGME accredited urology residency program are eligible for application.

The clinical training year provides a broad exposure to routine and complex endourologic conditions.  This exposure includes education in the work up and medical management of benign prostatic hyperplasia (BPH) and stone disease.  Clinical time will be with Drs. Rivera and Shelton as well as other clinical faculty at Methodist Hospital, IU North and IU West Hospitals. Surgical training during the clinical year includes adequate exposure to both minimally invasive treatment of stone disease and holmium laser enucleation of the prostate (HoLEP).  The fellow will have significant surgical volume to become adept at percutaneous renal access, advanced percutaneous nephrolithotomy techniques, advanced ureteroscopy, and HoLEP.  The fellow will also gain expertise at metabolic stone management.

Overall, total surgical volume at IU where the fellow will be participating faculty perform approximately 800 HoLEPs, 500 PCNLs and 600 ureteroscopies annually.  Typical case volumes for the clinical year for index cases is: ureteroscopy 200 cases, percutaneous renal access and PCNL 150-200 cases, and HoLEP 150-200 cases.  The fellow will spend time, if desired,  working with Drs. Williams and Lingeman in our basic science lab.  We have a robust lab with the ability to perform fellow driven studies with high efficiency.   The research training provides the fellow with a valuable educational experience that will result in peer-reviewed publications in endourologic journals.  Research training includes exposure to evidence-based medicine, clinical trial design, and grant writing. All fellows are encouraged to apply for funding through the American Urological Association (AUA) scholarship program based on a project they have a strong interest in pursuing.  During the second clinical year the fellow’s clinical duties will include running their own OR room at Methodist Hospital and Eskenazi, roughly one day per week. The Call Schedule and Patient Duties During the fellowship the fellow with take call once every 10-12 weekends and approximately 1 weekday in 12.  During the call the fellow will take call as supervisory staff.  The fellow will not be expected to take call in the month of July or August. Overview
Fellowship in Minimally Invasive Urology (Laparoscopy/Robotic Surgery)

The one or two year fellowship recognized by the Endourological Society involves comprehensive training in all aspects of minimally invasive urology with an opportunity for the two year fellows to graduate with a masters degree in clinical research from Indiana University. If qualified candidates for a two year fellowship are not available, applicants will be considered for a one year clinical training program. The focus of this training will be all aspects of advanced laparoscopy including robot assisted surgery. Applicants must be eligible for an unrestricted medical license in Indiana and must have completed residency/fellowship in the US or Canada. Annual surgical volume includes over 400 laparoscopic and robot assisted surgeries.

The fellow will function as a junior attending urologist at the County (Eskenazi) Hospital one day each week, where he/she will supervise residents in the operating room as well as the urology clinics. The fellow’s time will be equally distributed between clinical work and research during the entire fellowship. The fellow’s annual salary and benefits are equivalent to that of a senior resident/fellow at IU.

Clinical:
The variety of advanced laparoscopic surgery exposes the fellow to all aspects of the urologic laparoscopic surgery including laparoscopic donor nephrectomy, robot assisted prostatectomy with the da Vinci surgical system, adrenalectomy, and other routine renal surgeries. The fellow will work predominately with Dr. Sundaram, but will be an integral part of the Urology Department, working with other faculty during laparoscopic, robotic and endourologic procedures. Dr. Koch performs 4-6 robot assisted radical prostatectomies per week and Dr. Gardner has a practice in advanced robotic surgery. The fellow would therefore be exposed to different techniques during the fellowship. The fellow does not normally have on call responsibilities as a trainee or an attending, but may be requested to assist. Almost all the fellow’s time is spent with surgical training, education and research. The University Hospital has two daVinci Si Surgical systems with simulators and another daVinci S system for clinical work. With only five residents for eight adult faculty members at the University Hospital, there is adequate surgical volume for the residents and the fellow.

Education:
An advanced laparoscopic training facility is managed by the fellow, for training of medical students and residents. Third and 4th year medical students are offered a month long elective in urologic laparoscopy. The fellow is closely involved with their education and clinical research. The fellow does not have outpatient clinic responsibilities at the University Hospital. The da Vinci surgical system is used predominately by urologists.

Research:
A fully equipped animal laboratory is available in the Indiana University Medical Center. Several clinical studies are presently in progress. There are comprehensive MIS databases for prostatectomies, renal surgery and adrenalectomies.

Master of Clinical Research:
A two year Clinical Investigator and Translational Education (CITE) Program leading to a master of science in clinical research degree through the IU Graduate School, is available to qualified fellows. Our 2013-2015 fellow is presently enrolled in this unique program and is expected to graduate in 2015 with the fellowship and an MS in Clinical Research.

Description of the Fellowship
The Fellowship in Endourology provides specialized, comprehensive training in all aspects of endourologic Urologic Surgery under the mentorship of four fellowship trained Endourology Faculty: Drs. Nicole Miller, Jennifer Robles, Nicholas Kavoussi and Duke Herrell. This fellowship uniquely provides a breadth of exposure to holmium laser enucleation of the prostate (HoLEP), medical and surgical management of complex stone, as well as advanced laparoscopic/robotic surgery for reconstructive and oncologic applications. The fellowship is a one- or two-year program, including clinical and focused laboratory research.

Effective January 1, 2022, the Department of Urology will not sponsor H1-B or O-1 visa applications for fellows. All applicants must be able to obtain a full medical license in the state of Tennessee.

Clinical
Fellows will meet the standard requirements for certification by the Endourological Society. Typically, fellows participate in at least 150 endourologic procedures during each fellowship year, including > 50-60 percutaneous renal procedures, > 75 - 100 HoLEP procedures, and > 150 flexible or rigid ureteroscopy procedures. Additionally, fellows will have the opportunity to participate in ~ 50 laparoscopic and robotic urology procedures annually. The fellow will have, approximately, 1-2 days per week for research activity and 2-3 days per week for clinical experience. The fellow will be exposed to a variety of cutting-edge techniques, including percutaneous access to the kidney (fluoroscopic and ultrasound-guided), HoLEP, ureteroscopy with various laser modalities and novel sheaths, as well as advanced robotic reconstruction and retroperitoneal surgical approaches.

Research
Research projects are varied and include both basic science and clinical research opportunities. Fellows will be mentored to publish manuscripts in peer-reviewed journals and present research at meetings. Ongoing collaborative research is conducted through a variety of advanced collaborations including work with Vanderbilt University Departments of Biomedical Engineering, Biomechanical Engineering and Chemical Engineering. Additional research opportunities include outcomes and clinical research, epidemiologic and bioinformatics projects, quality improvement with extensive collaborations across Vanderbilt University, Vanderbilt University Medical Center and the Nashville VA Medical Center. Several ongoing projects are supported by NIH funding. Fellows can also access resources aimed at the development of entrepreneurship and intellectual property knowledge.

Education:
The fellow will also be responsible for preparation of teaching conferences for residents and staff. The fellow will participate in teaching residents and medical students in the evaluation and management of endourologic and laparoscopic urology patients.

o       Provide patient care including consult service, surgery and inpatient service under the supervision of the Medical Director


 

o       Teach hospital or practice personnel or visitors as requested.


 

o       Complete research projects as designated by the Director.


 

a     Take call as designated by the Director.


 

a     Bill in accordance with Medicare guidelines


 

a     Performs other duties as assigned or directed by the Program Director of appropriate Specialty and Subspecialty Residency Program to insure the smooth operations of the department and a complete educational experience.


 

 

Eligibility

    • If International, must be eligible for an H1B clinical visa. Florida Hospital cannot support J-1 visas



 o       H1B clinical visa requires completion of steps 1, 2 and 3 of the USMLE


 

    • Must be eligible for a State of Florida training license

    • Must be eligible for malpractice as determined by the Adventist Health System (AHS)Trust
